Registration is dependent upon the following conditions. Please read these carefully and only submit your application if you agree to these.

1. I agree that data I submit data will be accurate to the best of my knowledge. The quality of data submitted is my responsibility
2. I agree that any publication made using the data contained within this system will acknowledge the “BMP user group” as a co-author and include a list of participating institutions.
3. I agree that data I submit may be viewed by other registered users who will be medical practitioners and their staff in accordance with the above conditions.
4. I agree to inform the administration of any change in my status or place of work so that the list of current users can be updated promptly.
5. Patient anonymity must be observed at all times. If a legal action results from this rule not being observing will be the sole responsibility of the registrant who submitted the data.
